Emergency Department RN – Care Coordination

Location: Bronx, NY 10461
Contract: 13 Weeks
Hours: 40 per week | Day Shift 10 AM – 6 PM
Department: Emergency Services – Care Management

Weekly Gross: $2,779.87

Overview
This position supports Emergency Department operations through care coordination, discharge planning, and utilization management to improve patient throughput and transitions of care.

Responsibilities

• Manage care coordination for ED patients
• Conduct utilization reviews and discharge assessments
• Partner with clinical teams to reduce length of stay
• Arrange post-discharge services and resources
• Ensure documentation meets regulatory guidelines

Qualifications

• NY RN License (active)
• 5+ years clinical RN experience
• Experience in discharge planning and utilization review
• ASN/ADN required
• BLS required

Preferred

• Bachelor’s or Master’s degree
• Case Management background
