A large academic medical center and regional Level I Trauma Center with a freestanding children’s hospital in Savannah, GA is seeking an experienced RN or LCSW Manager – Case Management.
Reporting to the Director of Case Management, this role provides operational oversight and leadership for hospital-wide case management services. The Manager promotes patient-centered care through effective care coordination, appropriate resource utilization, quality outcomes, and patient throughput while ensuring compliance with organizational policies and regulatory requirements.
Provide day-to-day leadership, supervision, and administrative oversight of the case management team
Coordinate and manage hospital-based case management services across assigned areas
Ensure care coordination practices align with hospital policies, regulatory standards, and best practices
Monitor quality metrics, documentation, assessments, and staff interactions to maintain high standards of care
Serve as a clinical expert and subject matter resource for case management processes
Promote quality outcomes, efficient patient flow, and appropriate utilization of hospital resources
Support interdisciplinary collaboration to enhance care transitions and discharge planning
Bachelor’s Degree in Nursing or Master’s Degree in Social Work (MSW) – Required
3+ years of acute care clinical nursing experience or hospital social work experience – Required
2+ years of supervisory or leadership experience in an acute care setting – Required
InterQual experience – Preferred
Active Registered Nurse (RN) license in applicable state(s)
OR
Active Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) license in applicable state(s)
Case Management Certification (CCM) and/or InterQual (IQCI) Certification – Preferred
