An acute care hospital in New York City is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services to adult inpatient populations. This role focuses on psychosocial assessments, care coordination, crisis intervention, and discharge planning within a hospital-based, interdisciplinary care environment. The LMSW will work onsite and collaborate closely with clinical teams to ensure safe, effective, and patient-centered care.
Conduct psychosocial assessments for adult inpatient populations
Develop and implement individualized care and discharge plans
Provide patient and family counseling and crisis intervention
Coordinate discharge planning and referrals to appropriate community resources
Collaborate with physicians, nurses, and allied health professionals
Maintain accurate, timely, and compliant documentation per hospital policies
Participate in interdisciplinary rounds and case conferences
Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)
Hospital experience preferred
Master’s Degree in Social Work
Active New York State Licensed Master Social Worker (LMSW) license
Primary Source Verification required
Adult inpatient psychiatry experience preferred
Strong clinical assessment, communication, and documentation skills
