Job Summary

An acute care hospital in New York City is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services to adult inpatient populations. This role focuses on psychosocial assessments, care coordination, crisis intervention, and discharge planning within a hospital-based, interdisciplinary care environment. The LMSW will work onsite and collaborate closely with clinical teams to ensure safe, effective, and patient-centered care.


Key Responsibilities

  • Conduct psychosocial assessments for adult inpatient populations

  • Develop and implement individualized care and discharge plans

  • Provide patient and family counseling and crisis intervention

  • Coordinate discharge planning and referrals to appropriate community resources

  • Collaborate with physicians, nurses, and allied health professionals

  • Maintain accurate, timely, and compliant documentation per hospital policies

  • Participate in interdisciplinary rounds and case conferences


Required Experience

  • Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)

  • Hospital experience preferred


Certifications & Licensure

  • Master’s Degree in Social Work

  • Active New York State Licensed Master Social Worker (LMSW) license

    • Primary Source Verification required


Additional Requirements

  • Adult inpatient psychiatry experience preferred

  • Strong clinical assessment, communication, and documentation skills

  • Onsite position – no remote option available (0% remote)
