The Certified Nursing Assistant (CNA) will work as part of a multidisciplinary team to deliver high-quality, patient-centered nursing and rehabilitation services in a long-term care and subacute skilled nursing environment. This role requires strong clinical skills, flexibility, and a commitment to resident safety and dignity.
Provide direct resident care in accordance with individualized care plans
Assist with activities of daily living (ADLs), including hygiene, grooming, feeding, and mobility
Perform ROM exercises, ambulation, and transfer assistance
Monitor and document vital signs, intake/output, and bowel movements
Conduct skin checks and report changes in resident condition promptly
Respond to call bells in a timely manner
Assist with room changes, bed making, and post-mortem care as needed
Ensure residents are properly using adaptive equipment as ordered
Communicate effectively with nursing staff regarding patient status and concerns
Complete all required documentation per facility policy and state/federal regulations
Participate in shift reports and interdisciplinary communication
Maintain professionalism and participate in required in-services and training
Active New Jersey CNA license (permanent)
Minimum 2 years of CNA experience in an LTC or subacute skilled nursing facility
Must be willing to care for patients with a wide range of conditions, including COVID-positive patients (required)
Ability to float throughout the facility as needed (rehab, vent, post-op, COVID units, etc.)
Strong competency in hands-on patient care duties
Must successfully pass a facility-issued general competency exam during the first week
Experience working with geriatric and disabled populations
Required:
High School Diploma or equivalent
Completion of an approved CNA training program
Required:
NJ Certified Nursing Assistant (CNA) License
BLS (AHA)
