Shift Details:
Evening Shift: 12:00 PM – 8:30 PM (EST)
Weekend Requirement: 1 weekend per 4-week schedule
The Case Manager RN (Care Coordinator) facilitates the interdisciplinary plan of care with a focus on evaluating clinical appropriateness, medical necessity, admission status, level of care, and resource utilization. This role supports patient throughput, quality outcomes, and safe discharge planning while balancing optimal care delivery and efficient use of resources.
Perform comprehensive assessments of assigned patients’ medical and psychosocial needs.
Develop, document, and coordinate individualized case management plans of care, including discharge planning.
Lead interdisciplinary collaboration to manage care using criteria-driven processes for appropriate level of care and resource utilization.
Conduct and participate in interdisciplinary team meetings to plan, implement, and evaluate patient care.
Evaluate admissions and ongoing stays for medical necessity using approved criteria; escalate issues through the appropriate chain of command.
Assess observation patients for appropriateness of observation status.
Perform utilization management reviews and communicate findings to third-party payers.
Act as a liaison between physicians, patients/families, hospital staff, and external agencies.
Ensure compliance with regulatory requirements, ethics, compliance policies, and quality initiatives related to case management services.
Identify barriers to care and patient throughput; track trends and develop action plans for process improvement.
Facilitate safe and timely discharge planning, including post-discharge service coordination.
Advocate for high-risk and under-resourced patient populations by coordinating education, assistance programs, and community resources.
Promote cost-effective care without compromising patient safety or quality.
Document care coordination activities and professional recommendations accurately and timely.
Report patient safety concerns, significant medical errors, or unanticipated outcomes per established guidelines.
Participate in mandatory education, performance improvement initiatives, and ongoing professional development.
Maintain regular, punctual, and dependable attendance.
Perform other duties as assigned.
Minimum 3 years of recent Case Management experience in an acute care setting OR
3 years of acute care RN experience in one of the following areas:
Med/Surg
Telemetry
Neuro
ICU
PCU
Emergency Department
Case management experience in home health or insurance will be considered if the candidate has:
3 total years of acute care experience, and
At least 1 year of acute care experience within the last 5 years
Associate Degree in Nursing (ADN) or Diploma in Nursing required
Bachelor of Science in Nursing (BSN) preferred
Active Florida RN license required or valid compact license with Florida licensure obtained within required timeframe
Advanced Practice RN license acceptable if current and compliant
Certification in Case Management, Nursing, or Utilization Review preferred
Reports to the Director of Case Management
