Job Overview

healthcare facility is seeking an experienced RN Care Manager to support care management operations within the Emergency Department. This role focuses on care coordination, utilization review, and discharge planning to improve patient flow, reduce length of stay, and ensure safe transitions of care from the ED.

Responsibilities

  • Perform care coordination, utilization review, and discharge planning for ED patients
  • Collaborate with interdisciplinary teams to optimize patient outcomes and reduce length of stay
  • Support patient throughput and effective transitions of care from the Emergency Department
  • Identify barriers to discharge and coordinate appropriate post-acute resources
  • Ensure compliance with regulatory, hospital, and payer requirements
  • Maintain accurate documentation in clinical and care management systems

Required Qualifications

  • Active New York State Registered Nurse (RN) license (NYSED)
  • Minimum 5 years of clinical experience as a Registered Nurse
  • Demonstrated experience in Care Coordination, Utilization Review, and Discharge Planning
  • Associate Degree in Nursing (ASN/ADN) required
  • BLS certification required
  • Primary Source Verification of RN license and registration from NYSED

Preferred Qualifications

  • Bachelor’s or Master’s degree in nursing
  • Case Management Certification
  • Prior leadership or care management experience
