Duration: 13-Week Contract
Shift: Evening Shift | 3:00 PM – 11:00 PM
Pay Rate: $47–$55 per hour

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Licensed Practical Nurse (LPN) for a Long-Term Care facility in Falmouth, MA. This role provides direct resident care while supervising nursing assistants and coordinating daily nursing operations. The ideal candidate demonstrates strong leadership, clinical expertise, and a commitment to delivering high-quality, compassionate care in compliance with all regulatory standards.

Key Responsibilities

  • Supervise nursing assistants and oversee daily nursing activities in accordance with facility policies and regulatory requirements

  • Plan, organize, and coordinate shift services, programs, and resident care activities

  • Complete and maintain accurate documentation for resident admissions, transfers, and discharges

  • Communicate with physicians regarding resident status, treatment plans, and medical orders

  • Receive, transcribe, and ensure accurate implementation of physician orders

  • Administer medications and treatments safely, verifying resident identity prior to administration

  • Perform nursing procedures including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage care, blood draws, range-of-motion exercises, and end-of-life care

  • Collect and process laboratory specimens (urine, blood, sputum)

  • Monitor vital signs and observe resident condition changes

  • Ensure dietary compliance and monitor resident nutrition, including family-provided foods

  • Educate residents and families regarding care plans and health needs

  • Maintain resident confidentiality and comply with HIPAA and facility policies

  • Participate in facility surveys, audits, and inspections as required

Qualifications

  • Graduate of an accredited LPN program or accredited nursing degree

  • Current, unencumbered Massachusetts LPN license

  • Active BLS/CPR and AED certification

  • Minimum of 1–2 years of LPN experience; supervisory experience preferred

  • Experience with IV therapy, phlebotomy, and long-term care nursing procedures

  • Strong leadership, communication, and decision-making skills

  • Knowledge of nursing practices, medical procedures, and long-term care regulations

  • Ability to lift a minimum of 50 pounds and meet physical job requirements

  • Patient, professional, and compassionate demeanor when working with residents, families, and staff
