Licensed Practical Nurse (LPN) – Evening Shift

Location: Falmouth, MA
Contract Duration: 13 Weeks
Shift: Evening | 3:00 PM – 11:00 PM
Pay Rate: $50 – $55 per hour


Position Summary

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week contract assignment in a long-term care setting. The LPN will provide direct nursing care to residents while supervising nursing assistants, ensuring high-quality care in compliance with federal, state, and local regulations.


Key Responsibilities

  • Provide direct nursing care to residents in accordance with established policies and procedures

  • Supervise and direct nursing assistants and support staff during assigned shifts

  • Administer medications and treatments as ordered by physicians, ensuring proper resident identification

  • Accurately document nursing care, physician orders, treatments, and resident responses

  • Transcribe and implement physician orders into resident charts, medication records, and care plans

  • Monitor resident conditions and communicate changes to physicians and nursing leadership

  • Participate in resident admissions, transfers, and discharges, including required documentation

  • Make rounds with physicians and assist in care planning as needed

  • Perform clinical procedures including catheter care, tube feedings, wound care, suctioning, and specimen collection

  • Take and record vital signs and perform routine clinical assessments

  • Educate residents and family members regarding health care needs and treatment plans

  • Maintain confidentiality and adhere to resident rights and HIPAA guidelines

  • Participate in facility inspections and surveys as required

  • Promote a culture of service excellence, teamwork, and compassionate care

  • Perform additional duties as assigned


Required Qualifications

  • Graduate of an approved Licensed Practical Nurse (LPN) program

  • Current, active, and unencumbered Massachusetts LPN license

  • BLS / CPR and AED certification required

  • Ability to read, write, speak, and understand English
