Shift: Evening Shift | 3:00 PM – 11:00 PM
Schedule: Full-Time | 40 Hours/Week | Every Other Weekend & Holiday

Travel Rate: $1,974.50 per week

Job Summary

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) for an evening-shift assignment at a Skilled Nursing Facility in Falmouth, MA. This role offers a consistent schedule and the opportunity to deliver high-quality, resident-centered care in a supportive clinical environment.

Key Responsibilities

  • Supervise and direct nursing assistants, including shift planning and service assignments in compliance with state and federal regulations

  • Complete admission, transfer, and discharge documentation; carry out and document physician orders

  • Maintain accurate nursing notes reflecting resident care and treatment responses

  • Administer medications and treatments and perform nursing procedures (e.g., wound care, catheterization, tube feedings, vital signs)

  • Collaborate with physicians and interdisciplinary teams on care plans and rounds

  • Ensure availability and proper functioning of medical supplies and equipment

  • Collect lab specimens and perform routine testing as required

  • Provide education and support to residents and families while maintaining HIPAA compliance

  • Assist with resident and family orientation and support physician care plan reviews

  • Participate in surveys, inspections, and quality initiatives

  • Promote professionalism, teamwork, accountability, and compassionate care

Qualifications

  • Graduate of an accredited LPN program

  • Active, unrestricted Massachusetts LPN license

  • BLS/CPR/AED certification required

  • Strong knowledge of nursing practices and regulatory requirements

  • Demonstrated leadership and supervisory abilities

  • Ability to lift 50+ lbs. and meet physical job requirements

  • Strong communication skills and ability to handle challenging situations with empathy and professionalism
