Position Overview
We are seeking a licensed Physical Therapist (PT) for a contract assignment in a skilled nursing or rehabilitation setting. The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, and delivering therapy services aimed at improving mobility, strength, and functional independence.

Perform initial evaluations and ongoing assessments of patients’ physical conditions
Develop and implement individualized treatment plans based on patient needs and goals
Provide therapeutic interventions to improve strength, balance, mobility, and endurance
Educate patients and caregivers on exercises, safety, and functional movement strategies
Document evaluations, daily treatment notes, progress reports, and discharge summaries
Collaborate with an interdisciplinary care team to ensure high-quality patient outcomes
Monitor patient progress and adjust treatment plans as clinically appropriate
Maintain compliance with facility policies, professional standards, and regulatory requirements
Day shift: 8:00 AM – 4:30 PM
30-minute meal break included
Weekend coverage as part of the regular schedule
Active Physical Therapist (PT) license in the practicing state
Degree in Physical Therapy from an accredited program
Prior experience in skilled nursing, rehabilitation, or similar healthcare settings preferred
Strong clinical assessment, communication, and documentation skills
Ability to work independently and as part of a multidisciplinary team
