Position Overview
We are seeking a licensed Physical Therapist (PT) for a short-term contract assignment in a skilled nursing/rehabilitation setting. The Physical Therapist will evaluate, plan, and implement individualized treatment programs to help patients improve mobility, strength, and overall functional independence.

Evaluate patients’ physical conditions and develop personalized treatment plans
Provide therapeutic interventions to improve mobility, strength, balance, and endurance
Educate patients and caregivers on exercises, safety, and functional mobility
Document evaluations, progress notes, and discharge plans in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to ensure coordinated patient care
Monitor patient progress and modify treatment plans as clinically appropriate
Maintain compliance with professional standards, policies, and procedures
Day shift: 8:00 AM – 4:30 PM
Includes a 30-minute meal break
Coverage includes weekends as scheduled
Active Physical Therapist (PT) license in the practicing state
Degree in Physical Therapy from an accredited program
Experience in skilled nursing or rehabilitation settings preferred
Strong clinical assessment and documentation skills
Ability to work independently and as part of a multidisciplinary team
