Location: Flagstaff, AZ 86001
Type: Contract
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (8-hour shifts, no weekends)
Pay Rate: $40–$44 per hour

Position Overview:
"Navitas Healthcare, LLC" is seeking an experienced Wound Care Nurse (LPN) to provide comprehensive wound assessment, treatment, and prevention for residents in accordance with clinical best practices and facility policies. The role involves developing and implementing skin care programs, collaborating with interdisciplinary teams, and providing guidance and training to staff on wound care, ostomy care, and prevention protocols.

Responsibilities:

  • Perform wound assessments, reassessments, and management for all patients following facility policies and clinical guidelines.

  • Develop and implement skin care programs to prevent resident skin breakdown.

  • Participate in updating wound prevention, wound care, ostomy care, and incontinence policies and procedures.

  • Revise plans of care based on resident response to treatment and evaluate care effectiveness.

  • Assess and reassess patient pain levels; implement and educate on pain management strategies.

  • Serve as a clinical resource to nursing and facility staff, providing in-services and training related to wound care, ostomy care, and incontinence.

  • Communicate clearly and effectively with nursing staff and physicians.

  • Collaborate with interdisciplinary healthcare professionals to ensure comprehensive resident care.

  • Ensure all documentation meets current standards, regulatory requirements, and best practices.

  • Participate in ongoing training and continuing education to stay current on advances in wound prevention and care.

Requirements:

  • Current and active LPN or RN license to practice in Arizona.

  • Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN).

  • Ability to obtain and maintain a Fingerprint Clearance Card issued by the Arizona Department of Public Safety.

  • Current CPR certification obtained through in-person assessment.
