Job Title: Registered Nurse – Case Manager
Location: Syracuse, NY 13210
Contract Duration: 13 Weeks
Shift: Day Shift | 5x8s | 7:00 AM – 3:00 PM
Gross Weekly: $2,694/week
W2: $32/hr
Stipend: $1,414/week
The Registered Nurse – Case Manager is responsible for coordinating patient care, ensuring appropriate utilization of resources, and supporting safe and effective discharge planning. This role involves collaboration with interdisciplinary teams within an acute-care setting to optimize patient outcomes and ensure continuity of care.
Conduct comprehensive assessments to determine patient needs, discharge readiness, and barriers to care
Coordinate care transitions, discharge planning, and post-acute services
Perform utilization review to ensure appropriate level of care and compliance with payer requirements
Collaborate with physicians, nursing staff, social workers, and external providers to support patient care goals
Monitor patient progress and adjust plans of care as needed
Document assessments, interventions, and care coordination activities in the EMR
Ensure compliance with hospital policies, regulatory standards, and quality initiatives
Engage with patients and families to provide education, support, and resource navigation
Active Registered Nurse (RN) License in New York
BLS certification (required)
Minimum 1.5 years of recent acute-care case management experience (within the last 2 years)
Experience in large acute-care or trauma-level hospitals
Trauma Level I preferred
Trauma Level II required
EPIC EMR experience – required
Strong knowledge of utilization review, care coordination, CDI, and payer guidelines
