Location: Carmichael, CA
The Occupational Therapist (OT) will evaluate, plan, and implement therapeutic programs that enhance residents’ ability to perform daily living activities, restore functional independence, and improve overall quality of life. The therapist will work closely with an interdisciplinary team in a supportive rehabilitation environment.
Perform comprehensive assessments of residents’ functional abilities and rehabilitation needs.
Develop individualized treatment plans aimed at improving mobility, coordination, strength, and daily living skills.
Provide therapeutic interventions including ADL retraining, therapeutic exercises, and functional mobility training.
Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards.
Educate residents and families on adaptive techniques, equipment use, and home exercise programs.
Collaborate with nursing, physical therapy, speech therapy, and medical staff to ensure coordinated care.
Participate in care planning meetings and contribute clinical expertise to interdisciplinary discussions.
Monitor progress and adjust therapy plans based on functional outcomes and patient response.
Maintain compliance with all state licensure requirements and professional practice guidelines.
Promote a safe, patient-centered therapeutic environment focusing on functional independence.
Active and unrestricted Occupational Therapist (OT) license in the state of assignment.
Experience in skilled nursing or geriatric rehabilitation preferred.
Strong clinical reasoning, communication, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
