Location: Montesano, WA
The Speech-Language Pathologist (SLP) will provide comprehensive evaluation, diagnosis, and treatment for residents requiring speech, language, cognitive-communication, and swallowing interventions. The clinician will work in a collaborative, interdisciplinary environment focused on enhancing patient outcomes and promoting functional independence.
Conduct speech, language, voice, cognitive, and swallowing evaluations for residents.
Develop and implement individualized treatment plans to improve communication and swallowing function.
Document assessments, progress notes, and treatment outcomes in accordance with facility and regulatory standards.
Provide patient and family education on communication strategies, safe swallowing, and home exercise programs.
Collaborate with nursing staff, rehabilitation team members, and physicians to ensure coordinated patient care.
Participate in care planning meetings and contribute to interdisciplinary approaches for patient progress.
Monitor patient response to therapy and adjust treatment plans as needed.
Ensure compliance with all state, federal, and facility policies and professional guidelines.
Utilize therapeutic equipment and technology to support patient rehabilitation efforts.
Support a positive, patient-centered environment while promoting safety and quality of care.
Active and unrestricted SLP license in the state of assignment.
Experience in skilled nursing or geriatric care preferred.
Strong assessment, communication, and documentation skills.
Ability to work independently and collaboratively within a multidisciplinary team.
