Job Description
The Physical Therapist provides high-quality rehabilitative care to patients requiring physical therapy services in a post-acute or long-term care setting. The PT will work collaboratively with an interdisciplinary team to evaluate patient needs, develop individualized treatment plans, and support patients in achieving optimal functional outcomes.
Conduct comprehensive physical therapy evaluations and assessments.
Develop and implement individualized treatment plans based on patient needs and clinical best practices.
Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
Collaborate with physicians, nursing staff, and other rehabilitation professionals to coordinate patient care.
Document treatment sessions, progress notes, and care plans in accordance with facility and regulatory requirements.
Educate patients and caregivers on exercises, safe mobility techniques, and ongoing therapy needs.
Participate in discharge planning to ensure continuity of care.
Adhere to professional, ethical, and legal standards of physical therapy practice.
Provide therapy services at an affiliated nearby facility if required.
Active and unrestricted Physical Therapist license in the state of Washington.
Graduate of an accredited Physical Therapy program.
Minimum 1 year of recent PT experience in a skilled nursing, acute care, or similar setting preferred.
Strong clinical reasoning, documentation, and patient-care skills.
Ability to work independently and as part of a multidisciplinary team.
Flexibility to float to a nearby facility if needed.
