Hours: 08:00 AM – 4:30 PM
The Occupational Therapist (OT) will provide comprehensive rehabilitative therapy services to residents in a skilled nursing/long-term care environment. The primary focus is to restore functional independence, improve daily living skills, and support residents in achieving their highest level of self-sufficiency.
Conduct initial assessments and develop individualized treatment plans.
Provide therapeutic interventions to enhance ADLs, motor skills, coordination, and cognitive functioning.
Recommend adaptive equipment and modifications to support resident safety and independence.
Collaborate with interdisciplinary team members on care planning.
Maintain accurate and timely documentation per facility and regulatory guidelines.
Educate residents, family members, and staff on therapeutic techniques and safety strategies.
Active state-issued Occupational Therapist (OT) license.
Degree in Occupational Therapy from an accredited program.
Previous SNF/LTC experience preferred.
Strong clinical reasoning, documentation, and communication skills.
Ability to work the full weekly schedule.
