The Physical Therapist (PT) will provide skilled rehabilitative services to residents in a long-term care/skilled nursing setting. The role focuses on improving mobility, strengthening function, managing pain, and enhancing resident independence.
Conduct comprehensive evaluations and develop individualized treatment plans.
Deliver therapeutic interventions to improve strength, range of motion, balance, and mobility.
Document treatments accurately according to regulatory and facility standards.
Collaborate with the interdisciplinary care team to align therapy goals.
Educate residents, families, and staff on safe movement and mobility practices.
Maintain professional and ethical standards of practice.
Active state-issued Physical Therapist (PT) license.
Degree in Physical Therapy from an accredited institution.
Prior SNF/LTC experience preferred but not required.
Ability to work the full weekly schedule, including weekends.
Strong communication, documentation, and clinical assessment skills.
