Hours: 08:00 AM – 4:30 PM
The Physical Therapist (PT) will deliver high-quality rehabilitation services to residents within a skilled nursing environment, focusing on improving mobility, functional ability, pain management, and overall quality of life.
Perform evaluations and create individualized treatment plans for residents.
Provide therapeutic interventions to improve mobility, strength, and functional independence.
Document all therapy services accurately and in compliance with facility and regulatory requirements.
Collaborate with interdisciplinary team members to support resident care goals.
Educate residents, families, and staff on mobility and safety techniques.
Maintain clinical standards and follow all professional and ethical guidelines.
Active state license as a Physical Therapist (PT).
Degree in Physical Therapy from an accredited program.
Previous experience in a SNF or long-term care setting preferred.
Ability to work a full schedule, including weekend availability.
Strong communication, documentation, and clinical reasoning skills.
