The Occupational Therapist will provide high-quality rehabilitative services in a skilled nursing or rehabilitation environment. This position involves evaluating patients, developing treatment plans, and delivering therapeutic interventions aimed at improving functional independence and quality of life.
Perform comprehensive evaluations and create individualized care plans.
Deliver therapeutic interventions to enhance ADLs, mobility, and functional performance.
Document evaluations, treatments, and patient progress accurately and timely.
Collaborate effectively with interdisciplinary team members (PT, SLP, nursing).
Modify treatment plans based on patient needs and progress.
Participate in care planning meetings as needed.
Maintain compliance with state, federal, and facility guidelines.
Ensure safe and effective therapy delivery in a supportive care environment.
Active state license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA).
Prior experience in SNF/LTC or rehabilitation settings preferred.
Ability to commit to the full contract duration.
Strong clinical reasoning and documentation skills.
