Case Manager RN – Evening Shift (Full-Time, Permanent)
Location: Tallahassee, FL (32310)
Shift: 12:00 PM – 8:30 PM EST
Weekend Requirement: 1 weekend per 4-week schedule
Compensation: $31.94–$43.92/hr (experience-based)
Shift Differentials: $2.50/hr Evening | $2.00/hr Weekend (stackable up to $4.50/hr)
Sign-On Bonus: $10,000 (paid in 5 installments over 2 years)
Relocation: Considered on a case-by-case basis
"Navitas Healthcare, LLC" is seeking a skilled and compassionate Case Manager RN to support a diverse acute-care patient population through effective care coordination, patient advocacy, and collaborative discharge planning. This role is essential in ensuring timely patient progression, appropriate resource utilization, and high-quality clinical outcomes across the continuum of care.
Minimum 3 years of recent Case Management experience in an acute-care setting,
OR 3 years of recent acute care experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED.
Home health or insurance case management experience may be considered if the candidate has:
– At least 3 years total acute care RN experience, and
– At least 1 year of acute care experience within the last 5 years.
Associate Degree in Nursing or Nursing Diploma required; BSN preferred.
Active FL RN license or active compact RN license (must obtain FL license within 90 days if practicing under compact).
APRN licensure accepted if aligned with job responsibilities.
Professional certification in Case Management, Nursing, or Utilization Review preferred.
Perform comprehensive assessments of medical, psychosocial, and discharge needs.
Develop individualized case management plans and communicate goals with clinical teams.
Coordinate interdisciplinary care to support appropriate level of care and efficient patient throughput.
Lead and document interdisciplinary team meetings while maintaining regulatory and organizational standards.
Conduct utilization management reviews and communicate clinical information to third-party payors.
Serve as a liaison among patients, families, providers, and community agencies to ensure continuity of care.
Identify barriers to care and develop action plans to support safe and timely transitions.
Evaluate medical necessity and admission status at appropriate intervals, escalating concerns as needed.
Coordinate post-discharge services, community resources, and outpatient support.
Support care initiatives for under-resourced populations and recurring patient groups.
Accurately document case management activities and adhere to quality and compliance standards.
Participate in performance improvement initiatives and required education.
Report patient safety concerns and unanticipated outcomes promptly.
Perform other related duties as assigned.
