We are seeking a skilled and compassionate Occupational Therapist (OT) to provide high-quality rehabilitative care within a long-term care/rehabilitation setting. The therapist will evaluate patient needs, develop individualized treatment plans, and deliver effective therapeutic interventions to improve functional abilities, independence, and overall quality of life.
Conduct comprehensive patient evaluations and functional assessments.
Develop, implement, and adjust individualized treatment plans based on patient goals and clinical findings.
Provide therapeutic interventions to enhance mobility, strength, coordination, and daily living skills.
Collaborate with interdisciplinary teams including PTs, SLPs, nursing staff, and physicians.
Document patient progress and maintain accurate clinical records.
Educate patients and caregivers on exercises, adaptive techniques, and equipment usage.
Ensure compliance with all clinical, regulatory, and professional standards.
Participate in care planning, discharge planning, and interdisciplinary meetings as needed.
Active and unrestricted Occupational Therapist (OT) license in the state of assignment.
Minimum of 1 year of experience in a skilled nursing facility, rehab center, or similar care environment (preferred).
Strong understanding of rehabilitative techniques, therapy modalities, and patient-centered care.
Excellent communication, evaluation, and documentation skills.
Full-time schedule.
Weekend coverage may be required depending on facility need.
Training/orientation provided on-site.
