Job Description:
A dedicated Physical Therapist Assistant (PTA) is needed to support rehabilitative services in a skilled nursing environment. The PTA will work under the supervision of a licensed Physical Therapist to deliver high-quality, patient-centered therapeutic care aimed at improving strength, mobility, and functional independence.
Responsibilities:
Implement physical therapy treatment plans in accordance with the supervising Physical Therapist’s direction.
Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions.
Document treatment sessions, patient responses, and progress in compliance with facility and regulatory standards.
Communicate observations, patient concerns, and treatment updates to the Physical Therapist and interdisciplinary team.
Educate patients and caregivers on safe mobility techniques, home exercise routines, and functional strategies.
Maintain therapy equipment and ensure a clean, safe treatment environment.
Participate in interdisciplinary care planning to support holistic patient progress.
Modify activities as needed based on patient tolerance and clinical direction.
Assist in discharge planning by providing functional progress updates.
Maintain compliance with state, federal, and facility guidelines.
Qualifications:
Active Physical Therapist Assistant (PTA) license in New Hampshire (or eligibility prior to start).
Graduate of an accredited PTA program.
Experience in skilled nursing or rehabilitation settings preferred.
Strong communication, documentation, and patient-interaction skills.
Ability to work collaboratively within a multidisciplinary care team.
Flexibility to float to an affiliated facility when required.
