Job Description:
A dedicated Physical Therapist Assistant (PTA) is needed to support rehabilitative services in a skilled nursing environment. The PTA will work under the supervision of a licensed Physical Therapist to deliver high-quality, patient-centered therapeutic care aimed at improving strength, mobility, and functional independence.

Responsibilities:

  • Implement physical therapy treatment plans in accordance with the supervising Physical Therapist’s direction.

  • Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions.

  • Document treatment sessions, patient responses, and progress in compliance with facility and regulatory standards.

  • Communicate observations, patient concerns, and treatment updates to the Physical Therapist and interdisciplinary team.

  • Educate patients and caregivers on safe mobility techniques, home exercise routines, and functional strategies.

  • Maintain therapy equipment and ensure a clean, safe treatment environment.

  • Participate in interdisciplinary care planning to support holistic patient progress.

  • Modify activities as needed based on patient tolerance and clinical direction.

  • Assist in discharge planning by providing functional progress updates.

  • Maintain compliance with state, federal, and facility guidelines.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in New Hampshire (or eligibility prior to start).

  • Graduate of an accredited PTA program.

  • Experience in skilled nursing or rehabilitation settings preferred.

  • Strong communication, documentation, and patient-interaction skills.

  • Ability to work collaboratively within a multidisciplinary care team.

  • Flexibility to float to an affiliated facility when required.
