Job Description:
A skilled Occupational Therapist (OT) is needed to support patient care in a rehabilitation-focused skilled nursing environment. The OT will provide functional assessments, therapeutic interventions, and collaborative care to help patients regain independence and improve their quality of life.

Responsibilities:

  • Perform comprehensive occupational therapy evaluations to identify functional deficits and therapy goals.

  • Develop and implement individualized treatment plans based on patient needs and clinical findings.

  • Provide therapeutic interventions to improve ADLs, cognitive skills, motor coordination, and functional mobility.

  • Document assessments, treatment sessions, progress notes, and care plans in alignment with facility and regulatory guidelines.

  • Collaborate closely with an interdisciplinary care team to support holistic patient care.

  • Educate patients and caregivers on home exercise programs, adaptive techniques, and safety strategies.

  • Participate in care conferences and contribute to discharge planning.

  • Ensure a safe, effective therapeutic environment that supports patient progress.

  • Adjust treatment plans based on patient response and clinical judgment.

  • Maintain compliance with state and federal regulations, professional standards, and facility procedures.

Qualifications:

  • Active Occupational Therapist (OT) license in the state of Massachusetts (or eligibility prior to start).

  • Graduate of an accredited OT program.

  • Prior experience in a skilled nursing or rehabilitation setting preferred.

  • Excellent assessment, communication, and documentation skills.

  • Ability to work independently while collaborating effectively within a multidisciplinary team.
