Job Description:
A skilled Occupational Therapist (OT) is needed to support patient care in a rehabilitation-focused skilled nursing environment. The OT will provide functional assessments, therapeutic interventions, and collaborative care to help patients regain independence and improve their quality of life.
Responsibilities:
Perform comprehensive occupational therapy evaluations to identify functional deficits and therapy goals.
Develop and implement individualized treatment plans based on patient needs and clinical findings.
Provide therapeutic interventions to improve ADLs, cognitive skills, motor coordination, and functional mobility.
Document assessments, treatment sessions, progress notes, and care plans in alignment with facility and regulatory guidelines.
Collaborate closely with an interdisciplinary care team to support holistic patient care.
Educate patients and caregivers on home exercise programs, adaptive techniques, and safety strategies.
Participate in care conferences and contribute to discharge planning.
Ensure a safe, effective therapeutic environment that supports patient progress.
Adjust treatment plans based on patient response and clinical judgment.
Maintain compliance with state and federal regulations, professional standards, and facility procedures.
Qualifications:
Active Occupational Therapist (OT) license in the state of Massachusetts (or eligibility prior to start).
Graduate of an accredited OT program.
Prior experience in a skilled nursing or rehabilitation setting preferred.
Excellent assessment, communication, and documentation skills.
Ability to work independently while collaborating effectively within a multidisciplinary team.
