Job Description:
We are seeking a dedicated Physical Therapist (PT) to join a skilled nursing and rehabilitation environment. The ideal candidate will deliver high-quality, patient-centered therapy services focused on restoring function, improving mobility, and promoting overall wellness.
Responsibilities:
Conduct comprehensive patient assessments to determine functional needs and therapy goals.
Develop and implement individualized treatment plans based on clinical evaluations.
Provide therapeutic interventions to improve strength, mobility, balance, and functional independence.
Document patient progress, treatment notes, and care plans according to facility and regulatory standards.
Collaborate with an interdisciplinary team to ensure coordinated, holistic patient care.
Educate patients and families on exercise programs, safety strategies, and ongoing care needs.
Participate in care conferences, progress reviews, and discharge planning.
Adhere to facility protocols, professional standards, and state/federal regulations.
Maintain a safe and supportive therapeutic environment for all patients.
Utilize clinical judgment to modify treatment plans as patient needs evolve.
Qualifications:
Active Physical Therapist (PT) license in the state of Massachusetts (or eligibility to obtain before start).
Graduate of an accredited PT program.
Prior experience in a skilled nursing or rehabilitation setting preferred.
Strong clinical reasoning, communication, and documentation skills.
Ability to work independently and collaboratively in a fast-paced environment.
