Speech-Language Pathologist (SLP)

Job Description
We are urgently seeking a skilled and compassionate Speech-Language Pathologist (SLP) to support a post-acute rehabilitation facility. The ideal clinician will have strong assessment and treatment skills, work well within an interdisciplinary team, and provide patient-centered care to adults and geriatric patients.
Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations.
Develop individualized treatment plans based on patient needs and clinical findings.
Deliver evidence-based therapy focused on communication, cognition, and dysphagia management.
Document evaluations, daily notes, progress reports, and discharge summaries in accordance with facility and professional standards.
Collaborate closely with PT/OT, nursing, physicians, and interdisciplinary team members to ensure optimal patient outcomes.
Educate patients, families, and caregivers on therapy techniques, communication strategies, and safe swallowing practices.
Participate in care conferences and interdisciplinary meetings as needed.
Maintain compliance with ethical standards, professional guidelines, and state licensure requirements.
Active SLP license (state-appropriate).
CCC-SLP strongly preferred; CFs not accepted unless specified.
Experience in skilled nursing, post-acute, or geriatric rehab preferred.
Strong clinical documentation and communication skills.
Ability to adapt to fast-paced environments and diverse caseloads.
