Licensed Practical Nurse (LPN)
Location: Elkins, WV
Shift: Day Shift
Additional Requirement: Fridays–Sundays or flexible availability with every-other-weekend requirement
Job Description:
The Licensed Practical Nurse (LPN) will provide compassionate, high-quality nursing care in a long-term care and rehabilitation environment. Responsibilities include, but are not limited to:
Deliver direct patient care in accordance with individualized care plans and facility guidelines.
Administer oral, injectable, and topical medications per provider orders.
Monitor and document vital signs, patient conditions, and treatment responses throughout the shift.
Perform clinical procedures within scope, including wound care, catheter care, and dressing changes.
Support residents with daily living activities while maintaining dignity and safety.
Communicate effectively with RNs, physicians, and interdisciplinary team members to ensure coordinated care.
Maintain accurate and timely documentation per regulatory and facility standards.
Respond to changes in patient condition and escalate concerns appropriately.
Assist with admissions, transfers, and discharges as needed.
Uphold infection control practices, safety protocols, and professional nursing standards.
