Job Description:
We are seeking a committed and patient-focused Physical Therapist to provide rehabilitative services within a skilled nursing facility setting. The PT will perform evaluations, develop individualized treatment plans, and deliver therapeutic interventions aimed at improving mobility, functional independence, and overall quality of life for residents.
Responsibilities:
Conduct comprehensive evaluations and develop customized treatment plans for residents.
Provide daily therapeutic interventions targeting mobility, strength, balance, and functional improvement.
Document evaluations, progress notes, and treatment outcomes in accordance with professional and facility standards.
Collaborate closely with nursing staff, occupational therapy, speech therapy, and the interdisciplinary care team.
Educate patients, caregivers, and staff on mobility techniques, safe transfers, and exercise programs.
Recommend and adjust assistive devices and durable medical equipment as needed.
Implement fall-prevention and safety strategies throughout care delivery.
Participate in care plan meetings and contribute to discharge planning.
Monitor patient progress and revise treatment goals or approaches as needed.
Maintain a safe, supportive, and therapeutic environment for residents.
Certifications Required:
Active Physical Therapy License/Certification
BLS (AHA)
