Job Description:

• Perform initial evaluations and create individualized treatment plans
• Provide daily therapeutic interventions to improve mobility, strength, balance, and function
• Document evaluations, treatments, and progress notes accurately and on time
• Collaborate with nursing, OT, SLP, and IDT for coordinated patient care
• Educate patients, families, and staff on mobility, safety, and exercise programs
• Recommend and adjust assistive devices and DME
• Ensure patient safety and implement fall-prevention strategies
• Participate in care plan meetings and discharge planning
• Monitor patient progress and update goals or treatment plans as needed

Certifications:

• BLS (AHA)
• PT License/Cert
