Job Title: Case Worker
Location: Bronx, NY
Contract Duration: 25 Weeks
Schedule: 8:00 AM – 4:00 PM
Hours per Week: 40
Pay Rate: $40 – $42 per hour
Position Summary:
"Navitas Healthcare, LLC" is seeking an experienced Case Worker to provide essential social services and support to individuals and families in need. This role involves assessing client needs, developing service plans, determining eligibility for assistance programs, and offering counseling or investigatory support as required.
Responsibilities:
Provide social services to clients requiring public assistance, food support, medical assistance, or protective services.
Conduct assessments and develop individualized service plans.
Determine client eligibility for various assistance programs.
Perform counseling and investigative activities related to client care.
Maintain accurate documentation and ensure compliance with program guidelines.
Required Skills & Experience:
Minimum 3 years of relevant experience.
Education:
Bachelor’s Degree from an accredited college or university.
