Job Description
We are seeking a dedicated and experienced Speech-Language Pathologist (SLP) to provide high-quality therapeutic services within a skilled nursing/rehabilitation environment. The ideal candidate will be passionate about improving patient outcomes, capable of working independently, and committed to delivering person-centered care.
Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations.
Develop and implement individualized treatment plans tailored to patient needs.
Provide direct therapy focusing on speech, language, voice, fluency, and swallowing disorders.
Document evaluations, treatment sessions, progress notes, and discharge summaries in accordance with facility and professional standards.
Collaborate with interdisciplinary team members, including PT, OT, nursing, and physicians, to coordinate holistic care.
Participate in care plan meetings and provide clinical updates as needed.
Educate patients, families, and caregivers on communication strategies and home exercise programs.
Maintain compliance with all regulatory guidelines, documentation requirements, and professional standards.
Assist in maintaining a safe and supportive therapeutic environment.
Other duties as assigned.
Active state license as a Speech-Language Pathologist (SLP).
Master’s degree in Speech-Language Pathology from an accredited program.
Certificate of Clinical Competence (CCC-SLP) preferred but not always required.
Prior experience in a skilled nursing facility (SNF) or rehabilitation setting preferred.
Strong clinical assessment and documentation skills.
Excellent communication, interpersonal, and organizational abilities.
Ability to work independently and manage caseload efficiently.
