Job Description:
We are seeking an experienced and compassionate Physical Therapist (PT) for a travel assignment in a skilled nursing/rehabilitation environment. The PT will provide high-quality therapy services aimed at improving mobility, strength, functional independence, and overall quality of life for residents.
Evaluate residents to determine functional status and develop individualized treatment plans.
Deliver evidence-based physical therapy interventions to improve strength, mobility, balance, and endurance.
Provide restorative care and rehabilitation treatments in accordance with clinical guidelines.
Collaborate with interdisciplinary care teams, including physicians, nurses, occupational therapists, and speech therapists.
Document evaluations, progress notes, treatment sessions, and discharge summaries accurately and timely.
Educate residents, families, and caregivers on therapy goals, techniques, and safety strategies.
Maintain compliance with all regulatory, professional, and facility standards.
Assist with coverage needs as required due to census changes or staffing support.
Participate in care plan meetings and contribute to resident outcome improvement initiatives.
Active and valid Physical Therapist license in the state of assignment (or compact license where applicable).
Minimum 1 year of recent PT experience, preferably in SNF, rehab, or geriatric care.
Strong understanding of therapeutic techniques, functional training, and restorative interventions.
Excellent communication, documentation, and clinical decision-making skills.
Ability to work independently and collaboratively in a fast-paced clinical environment.
Prior experience in skilled nursing or long-term care settings.
EMR/documentation proficiency.
Strong adaptability and patient-centered care mindset.
