Job Description

We are seeking a dedicated and reliable Travel Licensed Practical Nurse (LPN) to join a skilled nursing and rehabilitation facility on a 13-week contract. The ideal candidate must be comfortable working independently, providing high-quality patient care, and managing a consistent 12-hour day-shift schedule.

Key Responsibilities

  • Deliver safe, effective nursing care in accordance with state regulations and facility policies.

  • Administer medications accurately and on time, following all medication-administration protocols.

  • Document patient care, treatments, and observations thoroughly and promptly in electronic or paper health records.

  • Monitor vital signs, recognize changes in patient conditions, and report concerns to supervising RN or provider.

  • Provide wound care, treatments, and general patient support as ordered.

  • Assist with admissions, discharges, and care transitions as needed.

  • Collaborate with nursing assistants, therapists, and interdisciplinary clinical teams to ensure continuity of care.

  • Ensure compliance with infection control standards and safety practices.

  • Respond to emergencies promptly and appropriately.

  • Maintain professionalism and support a positive, resident-centered environment.

Requirements

  • Active and unrestricted LPN license.

  • Minimum 1 year of recent LPN experience in a skilled nursing, long-term care, rehab, or similar setting.

  • Ability to work 12-hour day shifts (7:00 AM – 7:30 PM).

  • Strong clinical judgment and ability to work with minimal supervision.

  • Excellent communication, documentation, and patient-care skills.

  • Must be dependable and punctual with consistent attendance.
