Shift: Nights (7:00 PM – 7:30 AM)

Job Description
We are seeking an experienced Travel LPN to join a skilled nursing and rehabilitation facility on a night shift assignment. This role requires a dedicated nurse capable of delivering high-quality patient care with professionalism and reliability.
Provide safe and effective nursing care to residents during overnight hours.
Administer medications and treatments according to facility protocols and regulatory standards.
Monitor patient health conditions, record vital signs, and document all observations accurately.
Respond promptly to changes in patient conditions and notify supervising clinicians as required.
Assist with wound care, IV care (if applicable), and other clinical tasks as assigned.
Maintain accurate and complete documentation in the electronic medical record.
Collaborate with the interdisciplinary team to support patient care plans.
Ensure compliance with all facility policies, state regulations, and scope-of-practice standards.
Support residents with activities of daily living as needed.
Maintain a clean, organized, and safe patient care environment.
Provide compassionate and professional care at all times.
Active and unencumbered LPN license.
Minimum 1 year of recent LPN experience in long-term care, skilled nursing, or rehab setting (strongly preferred).
Ability to work 12-hour night shifts consistently.
Strong clinical judgment and documentation skills.
Excellent communication and teamwork abilities.
Must pass background check, health screening, and drug test per assignment requirements.
Previous travel LPN experience.
Experience working night shift in long-term care or rehab settings.
EMR proficiency.
