Schedule: Sun, Mon, Tue, Wed, Thu, Fri, Sat (Full-week schedule)
Location: Willows, CA
We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join a skilled nursing and rehabilitation facility for a short-term assignment. The ideal candidate will provide high-quality therapy services, focusing on improving communication, cognition, and swallowing abilities in residents.
Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing impairments.
Deliver speech therapy services in accordance with approved therapy plans and facility guidelines.
Document evaluations, progress notes, daily treatment logs, and discharge summaries in compliance with regulatory and facility standards.
Collaborate with an interdisciplinary team including nursing, physical therapy, occupational therapy, and social services.
Provide education to patients, families, caregivers, and facility staff regarding therapy goals and home programs.
Participate in care planning meetings and recommend modifications to treatment as needed.
Ensure safe and effective therapy interventions within a skilled nursing environment.
Maintain compliance with all state, federal, and facility policies and regulations.
Active and valid SLP license in the state of assignment.
Graduate degree in Speech-Language Pathology from an accredited program.
ASHA Certificate of Clinical Competence (CCC-SLP) strongly preferred.
Experience in a skilled nursing facility (SNF) or long-term care setting preferred.
Strong clinical, documentation, and communication skills.
Ability to work independently and manage a full caseload.
Full-week availability required (Sun–Sat).
