Job Description

We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join a skilled nursing and rehabilitation facility for a short-term assignment. The ideal candidate will provide high-quality therapy services, focusing on improving communication, cognition, and swallowing abilities in residents.

Key Responsibilities

  • Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing impairments.

  • Deliver speech therapy services in accordance with approved therapy plans and facility guidelines.

  • Document evaluations, progress notes, daily treatment logs, and discharge summaries in compliance with regulatory and facility standards.

  • Collaborate with an interdisciplinary team including nursing, physical therapy, occupational therapy, and social services.

  • Provide education to patients, families, caregivers, and facility staff regarding therapy goals and home programs.

  • Participate in care planning meetings and recommend modifications to treatment as needed.

  • Ensure safe and effective therapy interventions within a skilled nursing environment.

  • Maintain compliance with all state, federal, and facility policies and regulations.

Requirements

  • Active and valid SLP license in the state of assignment.

  • Graduate degree in Speech-Language Pathology from an accredited program.

  • ASHA Certificate of Clinical Competence (CCC-SLP) strongly preferred.

  • Experience in a skilled nursing facility (SNF) or long-term care setting preferred.

  • Strong clinical, documentation, and communication skills.

  • Ability to work independently and manage a full caseload.

  • Full-week availability required (Sun–Sat).

  • This is an on-site position; remote work is not available.
