Shift: Day Shift (8:00 AM – 4:30 PM)
Contract Length: 13 Weeks
Schedule: Sunday – Friday (Six days/week)
A skilled and compassionate Physical Therapist is needed for a full-time contract assignment in a post-acute/rehabilitation setting. The PT will provide comprehensive therapy services aimed at improving patient mobility, functional independence, and overall quality of life. This role requires strong clinical skills, excellent communication, and the ability to work effectively within an interdisciplinary care team.
Perform comprehensive patient evaluations to determine therapy needs and establish individualized treatment plans
Provide therapeutic interventions focused on strength, mobility, balance, posture, and functional ability
Document all evaluations, progress notes, and treatment sessions accurately and timely
Collaborate closely with nursing, therapy, and medical teams to coordinate optimal patient care
Educate patients and families on treatment plans, exercise programs, and safe mobility practices
Track and report patient progress; adjust care plans as needed
Participate in care planning meetings and interdisciplinary discussions
Ensure compliance with all facility, state, and federal regulations
Maintain a safe, clean, and organized therapy environment
Perform additional duties as assigned
Active and unrestricted Physical Therapist (PT) license
Prior experience in a skilled nursing facility, rehab, or post-acute setting preferred
Strong understanding of therapy assessment tools and treatment techniques
Excellent communication, documentation, and time-management skills
Ability to work independently and within a team environment
