Job Description
The Licensed Practical Nurse (LPN) provides high-quality nursing care and supports daily clinical operations within the facility. This role ensures safe, effective, and compassionate care to residents and collaborates closely with the interdisciplinary team to maintain optimal patient outcomes.
Provide direct patient care in accordance with facility standards and state LPN scope of practice.
Administer medications and treatments safely and accurately.
Document patient care, vital signs, and clinical observations in the medical record.
Support RNs and physicians with assessments, procedures, and care planning.
Assist with patient mobility, personal care, and daily living activities as needed.
Monitor patient condition and promptly report changes to supervising clinicians.
Maintain a clean and safe patient environment.
Communicate effectively with patients, families, and staff to promote continuity of care.
Follow all facility protocols, infection control standards, and regulatory guidelines.
Perform additional duties as assigned.
