Job Title: Licensed Practical Nurse – Night Shift (LPN)

Location - Prescott, AZ

Shift: 10:30 PM – 7:00 AM
Schedule: Days off flexible; part-time hours accepted
Job Description

The Night Shift LPN provides high-quality nursing care, medication administration, and overnight supervision to ensure resident safety and well-being. This role supports the interdisciplinary care team and maintains compliance with state regulations and facility policies.

Key Responsibilities

  • Administer medications accurately and safely according to all applicable laws, regulations, and facility policies.

  • Provide treatments and complete required documentation in a timely and compliant manner.

  • Document and communicate resident status, care needs, and changes in condition to the interdisciplinary team following HIPAA standards.

  • Perform resident assessments; prepare incident and infection control reports; participate in QAPI initiatives as assigned.

  • Supervise and support nursing assistants during the shift to ensure quality care and adherence to facility procedures.

  • Address work-related questions, resolve care concerns, and reinforce proper care techniques.

  • Assist with training nursing assistants as needed during the shift.

  • Advocate for resident rights, ensuring respect, dignity, and adherence to wishes and directives.

  • Maintain a professional demeanor with residents, families, staff, visitors, volunteers, and vendors.

  • Conduct assessments and report findings to physicians as indicated.

  • Initiate, review, and update resident care plans.

  • Document all care provided in medical records accurately and promptly.

  • Perform other duties as assigned.

Qualifications

  • Active LPN license (state-specific).

  • CPR/BLS certification preferred.

  • Strong communication and documentation skills.

  • Ability to work independently during overnight hours.

  • Open to new graduates—training and support provided.
