Job Description
The Occupational Therapist (OT) will provide rehabilitative services to help patients develop, regain, or maintain daily living and work skills. The OT will evaluate functional abilities, create personalized treatment plans, and deliver therapeutic interventions in accordance with professional standards and facility guidelines.
Conduct comprehensive OT evaluations to determine patient needs and functional limitations.
Develop and implement individualized treatment plans focused on ADLs, fine motor skills, cognitive skills, and functional independence.
Provide therapeutic interventions to restore mobility, coordination, strength, and daily functioning.
Document assessments, treatment sessions, progress notes, and discharge summaries accurately and promptly.
Educate patients and families on home programs, adaptive equipment, and functional strategies.
Collaborate with interdisciplinary team members including PT, PTA, SLP, nursing, and case management.
Adjust treatment approaches based on patient response and clinical judgment.
Ensure compliance with state OT practice standards and facility policies.
Travel as required to support float locations (Salinas and Kingsburg).
Active and valid Occupational Therapist (OT) license in the state of California (or eligibility to obtain).
Current CPR/BLS certification.
Experience in skilled nursing, rehab, long-term care, or acute care preferred.
Strong communication, documentation, and patient-interaction skills.
Ability and willingness to float to multiple assigned locations.
