Shift: Night Shift (7:00 PM – 7:30 AM)

The Travel Licensed Practical Nurse (T-LPN) provides skilled nursing care to residents within a long-term care and rehabilitation environment. The nurse supports daily clinical operations by delivering safe, high-quality care in accordance with facility policies, state regulations, and professional practice standards. This role works collaboratively with RNs, CNAs, and the interdisciplinary care team to ensure optimal patient outcomes.
Administer medications safely and accurately in accordance with facility policy and state guidelines.
Provide direct nursing care including treatments, wound care, and monitoring of resident conditions.
Document nursing interventions, observations, and resident responses in the medical record.
Assist with admission, transfer, and discharge processes.
Report changes in resident status promptly to the supervising RN or provider.
Support infection control practices and maintain a safe clinical environment.
Supervise and direct CNAs to ensure quality care and proper workflow.
Respond to emergencies and follow established protocols.
Collaborate with interdisciplinary teams to support care plans and promote continuity of care.
Provide compassionate, resident-centered care while upholding dignity, privacy, and respect.
Active and unrestricted LPN license (state of assignment or compact).
Minimum 1 year of recent LPN experience, preferably in long-term care or skilled nursing.
Strong medication administration and clinical assessment skills.
Ability to work independently during night shifts.
Excellent communication, teamwork, and documentation abilities.
Knowledge of state nursing regulations and long-term care standards.
Prior experience as a travel LPN.
Familiarity with electronic medical record (EMR) systems.
Experience in rehabilitation, post-acute care, or memory care settings.
