Job Summary
The Speech-Language Pathologist (SLP) provides diagnostic, therapeutic, and rehabilitative services to patients with communication, cognitive-linguistic, and swallowing disorders. This position supports residents within the Windsor Skyline Care facility, ensuring high-quality treatment plans and patient-centered care to improve functional outcomes.
Conduct evaluations of speech, language, voice, cognition, and swallowing disorders.
Develop individualized treatment plans (ITPs) based on clinical assessments and patient goals.
Provide direct therapy for speech, language, cognitive, and swallowing impairments.
Document assessments, treatment sessions, progress notes, and discharge summaries in the EMR.
Collaborate with the interdisciplinary care team, including nursing, PT, OT, and physicians.
Educate patients, families, and caregivers on therapeutic techniques and home programs.
Participate in care plan meetings and provide professional recommendations.
Monitor patient progress and adjust plans of care as needed.
Ensure compliance with facility policies, state regulations, and ASHA standards.
Maintain a safe and therapeutic environment for all patients.
Master’s degree in Speech-Language Pathology.
Active state SLP license (or ability to obtain before start date).
Certificate of Clinical Competence (CCC-SLP) preferred; CFY may be considered based on facility needs.
Previous experience in a skilled nursing facility (SNF), rehabilitation setting, or long-term care preferred.
Strong knowledge of dysphagia management and cognitive-communication disorders.
Excellent documentation, communication, and clinical reasoning skills.
Ability to work independently and as part of an interdisciplinary team.
Experience with geriatric populations.
Familiarity with EMR systems used in rehabilitation settings.
Ability to manage a diverse caseload efficiently.
