Shift: Night Shift (6:00 PM – 6:30 AM)
Assignment Length: 13 Weeks
Schedule: Thursday, Friday, Saturday
Location: Albuquerque, NM
The Licensed Practical Nurse (LPN) provides direct nursing care to residents during overnight hours within a skilled nursing and rehabilitation setting. This role is responsible for delivering safe and effective care, administering medications, monitoring residents’ conditions, and supporting the overnight clinical team to ensure high-quality service.
Administer prescribed medications and treatments in accordance with state regulations and facility policies.
Perform routine nursing assessments and document findings clearly and accurately.
Monitor resident conditions, identify changes, and promptly report to the charge nurse or provider.
Maintain proper documentation in the medical record, including interventions, assessments, and significant events.
Prepare incident and infection control reports; participate in quality assurance and performance improvement activities.
Supervise CNAs during the shift to ensure appropriate care delivery and adherence to facility procedures.
Support and mentor overnight staff; assist in resolving workflow and resident care questions.
Uphold resident rights, privacy, and dignity at all times.
Communicate effectively with residents, families, oncoming staff, and interdisciplinary team members.
Update and follow individualized care plans to ensure continuity and accuracy of care.
Maintain professional conduct and promote a positive care environment.
Perform additional duties as assigned.
Active and unrestricted LPN license in the state of assignment.
Previous experience in long-term care or post-acute rehabilitation preferred.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently and make sound clinical decisions during night shift operations.
Experience using electronic medical record (EMR) systems.
Prior experience working night shifts in skilled nursing or similar settings.
Leadership capability in supervising and supporting nursing assistants.
