Job Description
We are seeking a dedicated Licensed Practical Nurse (LPN) for a night-shift role in a skilled nursing environment. The LPN will provide high-quality resident care, administer medications, document clinical interventions, and support nursing assistants throughout the shift. This role requires professionalism, strong communication skills, and a commitment to maintaining resident safety, dignity, and rights.
Administer medications and treatments safely, accurately, and in accordance with all applicable laws, policies, and procedures.
Document and communicate resident care and clinical changes to the interdisciplinary team, ensuring HIPAA compliance.
Perform routine assessments and complete required documentation, including incident reports and infection control reports.
Participate in QAPI activities as assigned to support quality improvement initiatives.
Supervise and support nursing assistants to ensure quality resident care and adherence to facility policies.
Assist in resolving workflow concerns, answering questions, and reinforcing proper care procedures during the shift.
Act as an advocate for residents, respecting their rights, preferences, and directives.
Maintain a professional and compassionate demeanor when interacting with residents, families, coworkers, and visitors.
Perform assessments and communicate findings to physicians when necessary.
Initiate, review, and update resident care plans.
Document all care provided in medical records accurately and in a timely manner.
Perform other duties as assigned by the supervising nurse or facility leadership.
Active Licensed Practical Nurse (LPN) license (state-specific).
