Job Description
We are seeking an experienced Occupational Therapist (OT) for a contract assignment in a skilled nursing facility. The OT will be responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions that promote functional independence and improved quality of life.
Conduct comprehensive assessments of residents’ functional abilities and limitations.
Develop and implement individualized treatment plans based on clinical findings.
Provide therapeutic exercises, ADL retraining, cognitive interventions, and functional mobility training.
Document all evaluations, progress notes, and treatment outcomes in accordance with regulatory and facility standards.
Collaborate with interdisciplinary team members to coordinate and optimize patient care.
Participate in discharge planning and provide equipment or follow-up recommendations.
Educate residents and caregivers on adaptive techniques and home programs.
Ensure treatment areas and equipment are used safely and appropriately.
Active state license as an Occupational Therapist (OT).
Previous experience in a skilled nursing facility preferred.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
