Position Overview
We are seeking an experienced Physical Therapist (PT) for a 14-week contract in a skilled nursing and post-acute rehabilitation environment. The PT will evaluate patient needs, develop individualized treatment plans, and deliver high-quality rehabilitative care that promotes functional independence and improved mobility.
Conduct comprehensive patient evaluations and establish personalized therapy plans.
Deliver evidence-based therapeutic interventions, including strengthening, balance training, mobility enhancement, and functional rehabbing.
Document evaluations, progress notes, and discharge summaries in accordance with clinical standards.
Monitor patient response to treatment and adjust plans as needed.
Collaborate with nursing, therapy, and care teams to ensure coordinated patient outcomes.
Educate patients and families on home exercise programs, safety, and functional strategies.
Active and unrestricted Physical Therapist (PT) license in the state of practice.
Experience in skilled nursing, long-term care, or geriatric rehabilitation strongly preferred.
Strong assessment, documentation, and communication skills.
Ability to manage caseloads effectively and work both independently and within an interdisciplinary team.
