Job Overview
We are seeking a reliable and skilled Licensed Practical Nurse (LPN) to support St. Anthony Health & Rehab during this short-term assignment. This role is ideal for an LPN with experience in skilled nursing, rehabilitation, or long-term care settings. The nurse will provide high-quality patient care, support clinical teams, and maintain compliance with facility and state guidelines.
Deliver direct nursing care to residents in accordance with care plans, facility policies, and regulatory standards.
Administer medications and treatments as prescribed.
Monitor patient conditions and report changes to the RN or provider promptly.
Document assessments, treatments, and observations accurately and in a timely manner.
Support ADLs, mobility assistance, and restorative care activities.
Collaborate with interdisciplinary teams to ensure optimal resident outcomes.
Maintain a safe, clean, and compliant care environment.
Work primarily on Sunday, Friday, and Saturday as assigned.
Active and unencumbered LPN license in the state of practice.
Previous experience in skilled nursing, rehab, or long-term care strongly preferred.
Proficient in electronic charting and documentation.
Strong communication and clinical assessment skills.
Ability to work independently and efficiently in a fast-paced environment.
Dependable and comfortable working weekends as part of the schedule.
