Job Summary
A skilled and dependable Licensed Practical Nurse (LPN) is needed to support patient care in a rehabilitation and long-term care environment. This role requires strong clinical judgment, excellent communication skills, and the ability to provide compassionate care to patients with varying levels of acuity.
Deliver safe and effective nursing care under the supervision of an RN or provider
Administer medications, treatments, and assist with procedures
Monitor and document patient conditions, including vital signs and clinical changes
Support daily living activities, mobility assistance, and overall patient comfort
Maintain accurate and timely patient records per facility guidelines
Collaborate with the interdisciplinary care team to support treatment plans
Maintain a clean, safe, and therapeutic environment for patients
Active and unencumbered LPN license (state-specific)
Recent experience working as an LPN (6–12 months preferred)
Ability to work 12-hour day shifts
Strong charting, communication, and assessment skills
Reliability and ability to meet all scheduled shift commitments
