Job Title: Physical Therapist (PT) – Day Shift

Location: Hayward, CA
Contract Duration: 13 Weeks
Guaranteed Hours: 35 hrs/week

Position Overview

Seeking a skilled and compassionate Physical Therapist (PT) for a full-time day-shift contract opportunity in a skilled nursing/rehabilitation environment. The PT will be responsible for evaluating patients, developing treatment plans, and delivering high-quality therapeutic services to support functional mobility, reduce pain, and maximize independence.

Key Responsibilities

  • Conduct comprehensive evaluations to determine patient needs and functional limitations

  • Develop and implement individualized treatment plans focused on mobility, strength, and functional improvement

  • Provide skilled PT interventions including therapeutic exercises, manual therapy, gait training, balance training, and more

  • Accurately document assessments, treatments, and progress notes in the EMR

  • Collaborate with an interdisciplinary care team to ensure coordinated patient care

  • Educate patients, caregivers, and families on home exercises, safety, and mobility techniques

  • Ensure compliance with state, federal, and professional standards of Physical Therapy

Required Skills & Experience

  • Minimum 1 year of Physical Therapy experience (SNF/LTC experience preferred)

  • Strong clinical reasoning and assessment skills

  • Ability to manage a full caseload independently

  • Experience with electronic documentation/EMR

  • Excellent communication and teamwork capabilities

Education & Licensure

  • Doctorate or Master’s Degree in Physical Therapy

  • Active PT License in the state of assignment (or eligibility to obtain)

  • BLS/CPR Certification required
