Shift: 9:00 AM – 5:00 PM
Pay Rate: $55 – $60 per hour
Job Summary:
"Navitas Healthcare, LLC" is seeking a dedicated Licensed Master Social Worker (LMSW) to provide professional social work services, support clients through assessments and care planning, and coordinate with interdisciplinary teams to ensure high-quality care.
Responsibilities:
Conduct psychosocial assessments and develop individualized care plans.
Provide counseling, crisis intervention, and case management services.
Collaborate with healthcare providers, families, and community resources.
Maintain documentation in compliance with state and organizational standards.
Support patients and families with discharge planning and community resource linkage.
Qualifications:
Required Experience: Minimum of one (1) year as a Licensed Master Social Worker.
Required Education: Master’s Degree in Social Work (MSW) from an accredited institution.
Licensure: Valid Licensed Master Social Worker (LMSW) credential issued by NYSED with primary source verification.
Preferred Qualifications:
Experience in healthcare, behavioral health, or community services environments.
