Day Shift: 6:00 AM – 6:30 PM
Night Shift: 6:30 PM – 6:00 AM
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to provide high-quality nursing care in a healthcare facility setting. The ideal candidate will be responsible for administering medications, performing treatments, and monitoring the well-being of residents in accordance with established nursing standards and physician orders.
This is a PRN (as-needed) position, ideal for professionals seeking flexible hours in a collaborative and supportive environment.
Administer medications and perform treatments as prescribed by the physician, within the scope of practice and license.
(Includes oral, enteral, parenteral, and suppository routes; wound care; tracheostomy, gastrostomy, jejunostomy, and catheter care.)
Observe and report the effectiveness of medications and treatments.
Maintain adequate stock of medications and medical supplies.
Participate in receiving and giving detailed shift reports.
Communicate effectively with residents, families, and interdisciplinary care teams.
Complete initial admission assessments for new residents.
Assist in developing and maintaining Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care plans.
Maintain accurate clinical documentation on resident conditions, treatments, and progress.
Participate actively in the Quality Assurance and Performance Improvement (QAPI) program.
Attend and participate in staff meetings and ongoing training sessions.
Gerontological and Geriatric Nursing theory and practice.
Effects of the aging process and age-specific care approaches.
Team nursing practices and care coordination principles.
Pharmacology, medication administration, and safety protocols.
Federal, State, and facility regulations regarding practical nursing, medication handling, and resident care.
Strong observation, documentation, and reporting skills.
Ability to interpret and apply healthcare laws and facility policies.

Active and valid LPN/LVN license in the state of practice.
Current CPR/BLS certification (preferred).

Ability to work flexible PRN shifts (Day/Night).
