Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) – PRN Position

Location: [Confidential – Healthcare Facility]
Schedule: 12-Hour Shifts (PRN – As Needed)

  • Day Shift: 6:00 AM – 6:30 PM

  • Night Shift: 6:30 PM – 6:00 AM

Position Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to provide high-quality nursing care in a healthcare facility setting. The ideal candidate will be responsible for administering medications, performing treatments, and monitoring the well-being of residents in accordance with established nursing standards and physician orders.

This is a PRN (as-needed) position, ideal for professionals seeking flexible hours in a collaborative and supportive environment.

Key Responsibilities

  • Administer medications and perform treatments as prescribed by the physician, within the scope of practice and license.
    (Includes oral, enteral, parenteral, and suppository routes; wound care; tracheostomy, gastrostomy, jejunostomy, and catheter care.)

  • Observe and report the effectiveness of medications and treatments.

  • Maintain adequate stock of medications and medical supplies.

  • Participate in receiving and giving detailed shift reports.

  • Communicate effectively with residents, families, and interdisciplinary care teams.

  • Complete initial admission assessments for new residents.

  • Assist in developing and maintaining Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care plans.

  • Maintain accurate clinical documentation on resident conditions, treatments, and progress.

  • Participate actively in the Quality Assurance and Performance Improvement (QAPI) program.

  • Attend and participate in staff meetings and ongoing training sessions.

Knowledge & Expertise

  • Gerontological and Geriatric Nursing theory and practice.

  • Effects of the aging process and age-specific care approaches.

  • Team nursing practices and care coordination principles.

  • Pharmacology, medication administration, and safety protocols.

  • Federal, State, and facility regulations regarding practical nursing, medication handling, and resident care.

Skills Required

  • Strong observation, documentation, and reporting skills.

  • Ability to interpret and apply healthcare laws and facility policies.

  • Competency in developing and executing individualized care plans.

  • Excellent oral and written communication skills.

  • Interpersonal skills to foster positive relationships with residents, families, and staff.

Qualifications

  • Active and valid LPN/LVN license in the state of practice.

  • Current CPR/BLS certification (preferred).

  • Previous experience in long-term care, rehabilitation, or geriatric nursing is a plus.

  • Ability to work flexible PRN shifts (Day/Night).
